The European health card in Spain: what it covers
The European health insurance card entitles the holder to necessary state provided healthcare during a temporary stay, on the same terms as Spanish residents, but it does not cover treatment in private clinics, transport home or repatriation.

The short version

The common mistake is not failing to carry it. It is presenting it at the wrong kind of clinic and discovering it has no effect there.
Where it works
Only in public health system centres: health centres, basic units and hospitals in the public network, which in the Balearics means the regional health service. Presented there, it gives access to the care that becomes necessary during the stay, with the same co-payments residents face. In a private clinic it has no effect, even where that clinic treats publicly referred patients.

What necessary means
Care that cannot wait until you return, judged clinically and taking into account the expected length of the stay and the nature of the treatment. It includes ongoing management of chronic conditions requiring attention during the trip, such as dialysis or oxygen therapy, though those should be arranged before travelling because capacity in high season is limited.
What to carry
The physical card or the provisional replacement certificate, plus photographic identification. Without it, the centre may invoice the treatment and you then reclaim from the issuing body at home, a process that can take 2 to 6 months. Check the expiry date before travelling: validity is usually 2 years and renewal takes anything from a few days to 3 weeks depending on the country.
For someone spending long periods on the island, the card is no longer the correct route and registration for healthcare as a resident should be arranged.
What travel insurance adds
Private treatment, medical transport, repatriation and trip cancellation. A basic policy costs 3 € to 9 € per day and complements the card rather than replacing it. For a 10 day stay that is 30 € to 90 €, less than a single emergency consultation at a private clinic, which runs 90 € to 250 € for the consultation alone.
